ADMISSIONS & APPLICATION PROCESS
Step One
Request Admission Packet by calling Tamaira Jones at (302) 994-4434, ext. 237 or via email at tjones@mhdel.org or click on the Application and Financial Statement buttons to print a .pdf version of both forms. When completed, mail both forms to: Highfield, The Masonic Home of Delaware, Attn: Tamaira Jones, 4800 Lancaster Pike, Wilmington, DE 19807.
Complete the Application for Admission and Financial Statement.
Step Two
Financial Statement must be approved by the Chief Financial Officer.
Step Three
Medical Admission Packet must be completed. (not required for cottage residents)
Resident Fact Sheet must be completed.
Copies of the following documents are needed:
Social Security Card
Medicare Card
Supplemental Medical Insurance Cards, including
Prescription, Dental, Vision, etc.
Advance Directives and Medical/ Financial Power of Attorney
Drivers License, Auto Ins., Registration
(Only if resident will have a car onsite)
Applicant will meet with Medical Staff and/or Admission Staff.
Step Four - Day of Admission
- Resident Living Agreement Contract or Cottage Lease will be signed.
- Payment is due upon admission according to current fee Schedule.
